MIRIAD Art & Design Ethics Form 2015/16

The following document can be used to identify whether there are any ethical issues associated with your project and plan your response to any issues raised.

Please note: This interactive version of the ethics form requires Adobe Acrobat reader v10 or later to complete. You need to open the downloaded PDF document in Acrobat reader to use and save the file correctly. The document will allow you to type information directly onto the form and contains interactive mouse roll-overs and links to ethical examples and guidance.

Do not use Apple Macintosh Preview software, to open the PDF as it does not support the layers function in this interactive document. Preview will also not save it correctly and you risk losing your work.
